The Impact of Multiple Chronic Diseases in Detroit and Cleveland
The Addressing Cardiometabolic Health Inequities by Early PreVEntion in the GREAT Lakes Region, or ACHIEVE GREATER, Center is a proactive versus reactive approach to reducing overwhelming cardiometabolic health disparities and downstream Black-White lifespan inequality in Detroit and Cleveland, two uniquely comparable cities.
In Detroit the multiple chronic disease burden is found in the near Westside and Lower Eastside while Cleveland sees the burden focused where the city's Black population is most dense on the Eastside. Cardiovascular disease (CVD) has six primary risk factors:
- Hypertension
- Obesity
- Diabetes
- Kidney Disease
- High Cholesterol
- Smoking
